Pediatric Respiratory Red Flags in the ED

These signs should prompt urgent senior review and often resuscitation/admission:

  • Severe respiratory distress: marked recession, head bobbing, grunting, or use of accessory muscles.
  • Silent or very quiet chest in a child with asthma symptoms (impending respiratory failure).
  • Stridor at rest, especially with drooling, toxic appearance or inability to lie flat.
  • Central cyanosis (blue lips/tongue) or SpO2 persistently < 92% on room air.

Quick Pediatric Respiratory Algorithm for the ED

  1. Immediate Assessment
    • Apply ABC approach; assess airway patency and breathing effort.
    • Check SpO2, respiratory rate, heart rate and level of consciousness.
    • Give high-flow oxygen if hypoxic or in significant distress (as per local protocol).
  2. Stratify Severity
    • Mild–moderate distress: tachypnoea, mild recession, able to talk/feed.
    • Severe/critical distress: marked recession, silent chest, cyanosis, altered mental state.
  3. Identify Likely Syndrome
    • Wheeze predominant → asthma/viral-induced wheeze, bronchiolitis (younger child).

    • Sudden onset unilateral decreased air entry/hyper-resonance → pneumothorax or foreign body.
  4. Treat While You Investigate
    • Start bronchodilators and steroids for asthma where indicated.
    • Consider nebulised adrenaline and steroids for significant croup.
    • Give antibiotics early for suspected bacterial pneumonia/sepsis.
    • Do not agitate a child with suspected severe upper airway obstruction; call anaesthetics/ENT early.
  5. Decide Disposition
    • Admit if any red flags, ongoing oxygen requirement, poor oral intake, or unreliable follow-up.
    • Discharge well children with mild symptoms, clear diagnosis, inhaler/supportive therapy plan and safety-netting advice.
