Pediatric Respiratory Examination in the Emergency Department
A pediatric respiratory examination is essential for evaluating children presenting to the emergency department with respiratory symptoms. The aim is to identify respiratory abnormalities affecting the lungs, airways and chest wall, and to rapidly recognise life-threatening distress.
Inspection
Begin with inspection to observe breathing patterns, chest movements, and signs of respiratory distress:
- Assess breathing rate, rhythm, and depth for age.
- Look for retractions (subcostal, intercostal, suprasternal), head bobbing, or nasal flaring.
- Check for chest wall deformities or asymmetry of chest movement.
- Observe for cyanosis of lips, tongue, or fingers.
- Note level of consciousness and ability to speak/cry normally.
Palpation
Palpate the chest and neck areas to assess respiratory function:

- Sudden onset unilateral decreased air entry/hyper-resonance → pneumothorax or foreign body.

- Start bronchodilators and steroids for asthma where indicated.
- Consider nebulised adrenaline and steroids for significant croup.
- Give antibiotics early for suspected bacterial pneumonia/sepsis.
